The following are the documents required for new applications. Note that a different set of documents is required for renewals.

Notes
  • Submit application documents for which a form is required electronically wherever possible (e.g. FD, MO, CD-R, etc.)
    Erase the section entitled "Instructions for Filling in the Form" when you submit.
  • Use PDF, MS WORD, or MS EXCEL to submit the documents. Do not submit documents by e-mail.
  • If you need to change anything in the documents after you have submitted them, please submit them based on Section 6, "Changing in Reporting Matters".
  • If you are submitting to a Conforminity Assessment Body, follow any instructions regarding the documents stipulated by that bodies.
