Application and Changes in Reporting Matters

Target Entities

Private Enterprises which have signed a PrivacyMark Use Agreement with the Japan Information Processing Development Corporation (JIPDEC) pursuant to the PrivacyMark System (hereafter, "Target Entities") could apply.

Fees

Fees will not be collected for the time being. In case fee collection is to be commenced, it would be announced on the website of PrivacyMark System.

Application

Base on Article 41 of the Act on the Protection of Personal Information, PrivacyMark Accredited Private Enterprises may voluntarily choose to become Target Entity of an Authorized Personal Information Protection Organization.

Therein, a Private Enterprise that want to be a Target Entity must file "Application for Target Entity of an Authorized Personal Information Protection Organization" (Form 1 (72kb PDF)) to the address below.

Please note that name of Target Entity, Contact Information, telephone number, E-mail Address and URL are to be publicly announced in the "List of Target Entities" at JIPDEC's website.

Changes in Reporting Matters

When a PrivacyMark accredited Private Enterprise that is registered as Target Entity has to change in the items of the List(Contact point of its personal information consultation service), please file "Application for Changes in Reporting Matters regarding Personal Information Consultation Service for Target Entity of an Authorized Personal Information Protection Organization " (Form 2 (63kb pdf)) to the address below.

Necessary Items as Changes in Reporting Matters

Name of Target Entity, personal information consultation service (contact information), telephone number, E-mail Address and URL

In case a PrivacyMark accredited Private Enterprise would terminates its registration as a Target Entity, it must file "Application for Termination of Registration of Target Entity of Authorized Personal Information Protection Organization" (Form 3 (51kb pdf)) to the address below.

Notification of Affiliated Authorized Personal Information Protection Organization

Target Entity must clearly show the name of Authorized Personal Information Protection Organization it belong to and the contact information for handling of complaints (including a case where a response is issued without delay to such individuals).

When Target Entity registers itself with JIPDEC, the name of the Authorized Personal Information Protection Organization and the contact information for handling of complaints are as follows:
